GHP GOLD MEDAL

  1. Title - The award shall be known as The GHP Gold Medal (year)
  2. Terms of Reference - A person who has made an outstanding contribution to the practice and/or politics of hospital pharmacy at national level which would be recognised by the majority of the membership.
  3. Nominations - Nominators should be current members of Guild National Professional Committee. Nominations must include a CV of the nominee and short supporting statement explaining why the nominee meets the terms of reference, and should be sent to the GHP Administrator by 31st December.
  4. Adjudication - This will be made by an adjudicating committee comprising the Chair (President), Vice-Chair (Vice-President) and Chair of Practice Committee. This will take place at the January/February NPC meeting at which the announcement will be made.
  5. Presentation of the Award - To be presented on the occasion of the dinner at the following Annual National Conference.

  3. Nominations - Nominators should be current members of Guild National Professional Committee. Nominations must include a CV of the nominee and short supporting statement explaining why the nominee meets the terms of reference, and should be sent to the GHP Administrator by 31st December.
  4. Adjudication - This will be made by an adjudicating committee comprising the Chair (President) and all Regional Members. This will take place at the January/February NPC meeting at which the announcement will be made.
  5. Presentation of the Award - To be presented on the occasion of the dinner at the following Annual National Conference.
